When patients leave the hospital, they often need continued short-term care to recover completely. That's where we come in. We work with you to set a care plan to meet your goals and abilities.

 

Our medical care team includes physician specialists, registered nurses, registered dietitians, physcial and occupational rehabilitation therapists, social workers, and case managers. 

 

Our team treats a variety of conditions including post-stroke recovery, hip fracture/replacement, knee or other joint replacement, neurological illnesses including Parkinson's disease, cardiac or respitory ailments, pneumonia, and wound management.
